May 14-15: Treehouse will be closed for staff training. There may be a slight delay on request processing during these days.

June 16: The last day to submit requests for the program year.

June 29 – July 5: Treehouse will be closed for staff appreciation. The request form will be down during this time.

July 6: New program year begins. The request form will reopen.

Important Info

Please use the form below to request funding from Treehouse for activities, school fees, camp, hair care, and other academic or extracurricular needs. Fill out the form, click ‘Submit Your Request for Review’ and a Treehouse staff member will contact you to discuss your request.

Please note:

  • Treehouse can issue reimbursements by check only to youth, caregivers, and community partners. PayPal is no longer available. All reimbursements require a receipt showing date of purchase, amount paid, name of business paid, and purchase details. Treehouse cannot reimburse for services or items purchased more than 60 days ago. Reimbursement is not guaranteed until a request has been approved. See here for the full reimbursement policy.
  • Requests generally take 5-10 business days to process.
  • Treehouse can issue direct payments to the vendor by credit card or check only. Before making a request, please confirm with the vendor that they can accept payments in the form of a business check or by credit card.
  • Treehouse cannot set up automatic account withdrawals and does not guarantee payment for services rendered prior to approving a request.
  • In this form, the term “participant” indicates the person enrolled in Treehouse services.
